About the Role
As a Care Coordinator, you will be responsible for coordinating care services and ensuring that care visits are delivered efficiently, safely and in line with client requirements.
Your duties will include:
Planning and maintaining care staff rotas
Coordinating and scheduling client visits
Ensuring continuity of care wherever possible
Liaising with clients, families, care staff and healthcare professionals
Monitoring service delivery and responding to changes in client needs
Supporting recruitment, onboarding and training activities
Maintaining accurate records and electronic care systems
Ensuring compliance with company policies and regulatory requirements
Supporting the on-call service as required
What We Are Looking For
Previous experience in a Care Coordinator role or similar position within Health & Social Care.
Experience using electronic rostering or care planning systems.
Excellent organisational and time management skills.
Strong communication and interpersonal abilities.
Ability to work effectively in a fast-paced environment.
Good knowledge of domiciliary care services and client-centred care.
NVQ Level 3 in Health & Social Care (desirable).
